- Statement letter IDR 6,000 stamped
- KRS Programming Application addressed to the Assistant Dean I
- Proof of Payment for Odd Semester Tuition Fees for the 2016/2017 Fiscal Year
Outside these dates are not served and must submit Academic Leave no later than September 27, 2016.
