PENCEGAHAN PENYAKIT TIDAK MENULAR DALAM PERSPEKTIF PUBLIC VALUE MANAGEMENT : STUDI PADA PUSKESMAS SINGOSARI KABUPATEN MALANG

Penelitian ini mengkaji Pencegahan Penyakit Tidak Menular (PTM) di
Puskesmas Singosari Kabupaten Malang dalam perspektif Public Value
Management (PVM) dengan menggunakan kerangka strategic triangle dari Mark
Moore, yang mencakup dimensi penciptaan nilai publik (public value), legitimasi
dan dukungan (legitimacy and support), serta kapasitas operasional (operational
capacity). Latar belakang penelitian dilandasi oleh meningkatnya prevalensi PTM,
seperti hipertensi, diabetes mellitus, kanker, dan penyakit jantung, yang
menimbulkan beban sosial, ekonomi, dan kesehatan masyarakat. Data Riskesdas
2018 serta laporan Profil Kesehatan Jawa Timur 2023 menunjukkan tren
peningkatan signifikan kasus PTM, yang mendorong pentingnya strategi
pencegahan melalui layanan primer di puskesmas.
Hasil penelitian menunjukkan bahwa Puskesmas Singosari telah
menjalankan upaya pencegahan PTM melalui program promotif dan preventif,
seperti deteksi dini faktor risiko melalui Posbindu PTM, penyuluhan kesehatan,
kegiatan edukasi berbasis masyarakat, serta layanan konseling. Upaya tersebut
memberikan kontribusi dalam meningkatkan nilai publik, khususnya berupa
peningkatan pengetahuan, kesadaran, dan akses masyarakat terhadap layanan
kesehatan. Namun demikian, implementasi program pencegahan PTM masih
menghadapi sejumlah tantangan. Dari segi public value, manfaat program sudah
dirasakan sebagian masyarakat, tetapi belum merata akibat keterbatasan cakupan
dan masih rendahnya partisipasi dalam skrining rutin. Dari sisi legitimacy and
support, dukungan kebijakan dari Dinas Kesehatan Kabupaten Malang cukup kuat,
namun koordinasi lintas sektor (pemerintah desa, sekolah, organisasi masyarakat)
belum optimal. Sementara itu, dalam aspek operational capacity, Puskesmas
Singosari memiliki SDM dan sistem layanan yang relatif baik, tetapi keterbatasan
sarana-prasarana, beban kerja tenaga kesehatan, serta keterbatasan anggaran masih
menjadi kendala utama. Temuan penelitian ini menegaskan bahwa keberhasilan
pencegahan PTM tidak hanya ditentukan oleh kapasitas internal puskesmas, tetapi
juga sangat bergantung pada dukungan eksternal dan partisipasi masyarakat.
Inovasi berbasis teknologi, komunikasi publik yang lebih persuasif, serta kolaborasi
multi-stakeholder menjadi faktor penting dalam memperkuat efektivitas program
pencegahan PTM di masa mendatang.

This study examines the prevention of Non-Communicable Diseases (NCDs)
at the Singosari Community Health Center in Malang Regency from the perspective of
Public Value Management (PVM) using Mark Moore's strategic triangle framework,
which includes the dimensions of public value creation, legitimacy and support, and
operational capacity. The background of this study is based on the increasing
prevalence of NCDs, such as hypertension, diabetes mellitus, cancer, and heart disease,
which impose a social, economic, and health burden on the community. Data from the
2018 Basic Health Research (Riskesdas) and the 2023 East Java Health Profile report
show a significant upward trend in NCD cases, which highlights the importance of
prevention strategies through primary services at community health centers.
The results of the study show that the Singosari Community Health Center has
implemented NCD prevention efforts through promotional and preventive programs,
such as early detection of risk factors through the NCD Posbindu, health education,
community-based educational activities, and counseling services. These efforts have
contributed to increasing public value, particularly in the form of increased knowledge,
awareness, and community access to health services. However, the implementation of
NCD prevention programs still faces a number of challenges. In terms of public value,
the benefits of the program have been felt by some members of the community, but
they are not yet evenly distributed due to limited coverage and low participation in
routine screening. In terms of legitimacy and support, policy support from the Malang
District Health Office is quite strong, but cross-sector coordination (between village
governments, schools, and community organizations) is not yet optimal. Meanwhile,
in terms of operational capacity, the Singosari Community Health Center has relatively
good human resources and service systems, but limitations in infrastructure, the
workload of health workers, and budget constraints remain major obstacles. The
findings of this study confirm that the success of NCD prevention is not only
determined by the internal capacity of health centers, but also highly dependent on
external support and community participation. Technology-based innovations, more
persuasive public communication, and multi-stakeholder collaboration are important
factors in strengthening the effectiveness of NCD prevention programs in the future.
