Submission of the Supplementary Examination is done by filling out the form <a href=\"https:\/\/docs.google.com\/forms\/d\/1hGxciHSxAsYjKNhWCRuumDR-HvouRQy022esez7YQlI\/edit\">attached<\/a>, by enclosing a letter of application compiled by yourself, accompanied by evidence that can be accounted for.
